»What to Expect
 

Intake
Call 740-6700 and ask for the clinic coordinator. We will talk to you about your concerns and the child’s presenting symptoms. An intake packet will be mailed. Your child’s name will be confirmed on the waiting list for evaluation once the completed forms are returned to our office.

Appointments
Two appointments will be scheduled for your child, usually with 4-6 weeks advance notice. We will begin a series of steps to collect current and historical information about your child.

  • Appointment #1
    Interviews, physical exam, testing and data collection.
  • Appointment #2
    Testing/data collection completed. Diagnosis and recommendations presented to family, legal guardian and others invited by the family.

Reports
Families leave the clinic with a three-page report explaining the diagnosis and intervention recommendations. The final report is mailed three to four weeks after appointment #2.

Medical Assistance usually covers this evaluation. Insurance will be billed.
